Notice of Termination
Effective June 3, 2009

Southpark Community Hospital is a hospital provider located at 314 Youngsville Highway, Lafayette, Louisiana 70508. Effective June 3, 2009, the hospital terminated all operations.

 

Please read the following regarding the medical records:

 

  1. Medical records will be kept in Spring, Texas in a secure and climate controlled facility.
  2. Patients may secure a copy of their medical records by a request either in writing by sending a fax to 832.717.3445 or sending an email to info@spacquisitions.com

Written requests must include a copy of a photo identification such as a drivers license and include a telephone number.  There will be a charge of $1.00 per page for first 25 pages, $.50 for 26-500 and $.25 thereafter, $15.00 handling charge and actual postage/delivery charge that will depend upon the method selected by the patient. There is a $10. fee for copies of x-rays, microfilm and electronic & imaging media.

 

3. Copies of the medical records will be sent via First Class mail or courier, as requested, within 15 working days of the receipt of the request and payment.
